Captain Luke Danville held his nerve to score the decisive penalty to send Hemsworth through after an epic battle with unlucky local rivals Glasshoughton.
Danville’s successful kick in the shootout put the Wells 3-2 up and it was vital as Mike Brearley then missed his attempt to end a tie that lasted over 240 minutes.
Saturday’s original clash also went to extra-time and it was inevitable that the replay would too.
Glasshoughton more than matched their higher league opponents and they deserved their tenth minute lead, given to them by Richard Collier.
Hemsworth, who have several of their star players on the treatment table, lacked bite going forward. It was noticeable too that the likes of Nash Connolly, Billy Law and Adam Wilson were missing.
Dom Blair did equalise for the Wells after breaking clear, towards the interval.
The second half was non-eventful and it wasn’t really until the end of extra-time that the game livened up. In the space of a few minutes, Collier hit the bar for Glasshoughton and Hemsworth’s Daniel Henighan watched his goal-bound effort get cleared off-the-line.
The penalties saw Nick Guest miss for Hemsworth, but Del Pollock, Brice Tiani and Danville score. Adam Hayton, Conor Glavin and Brearley failed to score for Welfare, while Nathan Keightley and Ryan Poskitt did score.
